To Request Services · Step 1: Meet with the Secretary for Disability Services to discuss eligibility for accommodations. · Step 2: Complete an intake and other forms prior to receiving services. · Step 3: Provide documentation to Disability Services. · Step 4: Student would pick up accommodation letters to be delivered to each professor and other campus personnel as needed. · Step 5: Meet with Carolyn Crawford, Director of Disability Services, to discuss extenuating circumstances · Step 6: Student would meet with each instructor to discuss the accommodations requested by Carolyn Crawford, Director of Disability Services Each client is required to register at the beginning of EACH SEMESTER in order to receive services from Disability Services. In addition, the following forms must be completed prior to receiving services.
